General Request of the Registrar Use this form to file a change of contact information, request an evaluation of your program, unofficial transcript, verification of enrollment or to make another request not covered by other forms and petitions.

Academic Enrollment Online Form or Academic Enrollment PDF Form This form is used to enroll in classes each quarter. To fill out the fields appropriately the student must consult the current class schedule and the Schedule of Tuition and Fees. To enroll in classes: download this form, print out the current class schedule and the Schedule of Tuition and Fees, fill in the fields with accurate information, fax the form to the registrar, and call the Student Finance Office (818-779-8424) to pay the tuition. Note: Filling out and faxing the form does not mean you are registered for classes until you call the Student Finance Office and pay for your classes.

Affirmation Statements Each student is required to read and sign this document affirming agreement with the school's mission, purposes and policies.

Application for Degree This form must be filled out and submitted to the registrar office in order to receive a certificate or bachelors degree. Download this form by clicking the link above. Print out the form. Fill in the fields according to the instructions and mail or fax the form to the Registrar, 14800 Sherman Way, Van Nuys, CA 91405, fax: 818-779-8428.

Distant Student Recommendation for Proctor Use this form to request a proctor. Students who live at a distance of more than 75 miles from the Van Nuys campus (referred to as Distant Student) can take final examinations with a proctor. The final examinations will be administered by the proctor on the same day that the final examinations are administered to local students on the Van Nuys campus.

Doctrinal Statement and Standard of Conduct All Students are required to sign the doctrinal statement and standard of conduct each quarter they are enrolled. If you have not done so this quarter please download the form, print it, sign it and fax it to the registrar.

Drop Add Schedule
If you need to drop a class, add a class, or change the status of the class from credit to audit or from audit to credit, please download this form, print it out, complete it and fax it to the registrar. Be sure to note any instructions on the form particularly in regard to fees.

Petition for Directed Study This form is to be used to request approval for the development of and the enrollment in a course not available in the current Schedule of Classes. If you need a class that is not currently available, download the form, print it, fill in all fields with accurate information, and fax the form to the registrar.

Petition for Incomplete This form is to be used to request approval for a deviation from established school policies. If for some reason such as prolonged illness you necessitate an incomplete for a class grade, please download the form, print it, fill in all fields with accurate information, and fax the form to the registrar.

Petition for Late Examination This form is to be used to request permission to take an examination, test, or quiz at a time that deviates from the established or regularly scheduled testing times. If you have a valid reason, such as surgery, critical illness, or previously scheduled events like a funeral, for why you cannot take an exam at its scheduled time, please download the form, print it, fill in all fields with accurate information, and fax the form to the registrar. Be sure to note any instructions on the form particularly in regard to fees.

Petition for Policy Exception This form is to be used to request approval for a deviation from established school policies. Students may petition to have stated policies modified in a given situation due to extenuating circumstances. If this the case, please download the form, print it, fill in all fields with accurate information, and fax the form to the registrar. Be sure to note any instructions on the form particularly in regard to fees.

Petition for Withdrawal Students wishing to withdraw from the school must complete a Petition to Withdraw, secure the necessary signatures, and submit it to the Registrar’s Office. Courses must be officially dropped or a grade of F will be entered on the permanent record. If you wish to withdraw from the college, please download the form, print it, fill in all fields with accurate information, and fax the form to the registrar. Be sure to note any instructions on the form particularly in regard to fees.

Petition for Program Change—Graduate This form is intended for students wishing to change from one degree program to another.

Release of Student Information This form allows students to permit The King's Seminary to disclose information such as grades, account balance, etc., to his/her parent(s), legal guardian(s), company, or another individual. This consent shall be valid throughout the student’s enrollment, but may be modified or rescinded in writing by the student. Information provided to parent(s), legal guardian(s), company, or another individual is for their use only and should not be disclosed to third parties without the student’s authorization.

Request for Transcript Any student requiring a transcript of his/her grades must fill out and submit this form. The King's College is required by law to receive a signed document from the student requesting the transcript before the transcript can be sent. If you are requesting your transcript, please download the form, print it, fill in all fields with accurate information, and mail the form to the registrar. Be sure to note any instructions on the form particularly in regard to fees.
